Supplemental recommendation. Just use common app.?

<p>I am having my advisor for one of my extracurricular activities write a recommendation letter. I have supplied him with the common app. teacher evaluation forms as a guideline. Should I submit the letter only with a post-note explaining that it is a supplementary recommendation? Or should I include the entire common app. teacher evaluation (including the check boxes for personal qualities)?</p>

<p>You don't need to note the rec...just make sure the recommender uses your name, etc...when he/she sends it out.</p>
