Supplemental Recs and Addtl. Info

Two (main) Questions:
1. How should I submit a supplemental recommendation (in this case from an employer)? Should I give it to my guidance counselor and ask her to send it along with the teacher recs, or should I submit it under additional information? If the former, should I check off yes for "will you be submitting supplementary material", or does that only apply to art, music, etc.
2. If I have any awards that I don't find particularly "meaningful" but still look good, should I submit them under additional information? I mean national merit semi, AP Scholar, my state scholar, etc. Would AP scores go under additional information as well? Does anyone know or at least have some idea of whether the additional information page is intended for things like this, or is usually used for other purposes?
Really, any info on supplemental recs and "additional information" would be much appreciated.
Thanks.

I had this same question a few months ago and I called admissions. They said that I should have my employer send in the rec him/herself in a sealed envelope addressed to Undergrad Admissions. They said it wasn't necessary for him/her to include your full name, SS#, etc. but I didn't believe that part. Also, they said that the employer should wait until Columbia has you on file.
