<p>I interned at a financial company for the past two summers and I am going to ask my boss for a letter of recommendation. I was wondering if there was a specific form for supplementary recommendations like the Teacher Evaluation for teachers, Secondary Report for counselors, and etc. I couldn't seem to find one on the common app form section.</p>
<p>Do I just ask my boss to write one for me and have him just mail the letter by itself to the colleges I am applying to?</p>
<p>A think a simple letter on letterhead that has much of the same information as is asked for on the teacher recommendation form would be appropriate. For example, it should be signed, include their title, a contact phone number and email, and the student’s full name, etc. </p>
<p>The goal of course is to provide insight that the college cannot obtain from the application materials.</p>
<p>Too many people make a mistake of trying to find a “big name” to give them a college recommendation. One admission officer said “The more famous the person making the recommendation, the less they know about the student.”</p>
<p>^ I agree with the above. Watch out for the big names, that can actually be a detriment to your app, as it will annoy admissions officers. Unless, of course, the big name knows you VERY well…</p>
<p>i did a similar thing sending in a letter of rec from a lawyer who i interned for. I emailed the school im applying to early decision (penn) and asked them what to do and they said have them send it to their admissions office with your name, number, address, social security number, and stuff like that attached to the letter. no extra supplementary forms necessary</p>
