Are they the same thing?

The teacher eval. is the form the teacher fills out with basic information about himself and the matrix that most schools have where he ranks you in certain areas. the recommendation letter is what the teacher attaches to the evaluation giving a more specific idea of what he thinks of you.

can the evaluation be on a separate form? Like just attached to another piece of paper when you mail it?

You can print the teacher evaluation form from the common app, have a teacher fill it out, and mail it together with the recommendation letter

say if i send in just the letter of rec without the teacher evaluation is that fine? or do i need to send colleges both Commonapp teacher evaluation along with my letter of rec

My two major teachers (English and Social Studies) are sending out the teacher evaluation plus their own letters of recommendation. I also have one outside letter from a previous teacher at my old school (who I’ve known for many years now) and she is only sending the letter. My guidance counselor said this was generally acceptable because it isn’t a main letter… You know what I mean? Haha. It wouldn’t fly if I’d let my English teacher do it, or my Social Studies teacher. But my outside recommender could be safe.
