<p>So one of my recommenders is doing my rec letter traditionally (offline) and I see that there is something called the Teacher Evaluation form. She already wrote a regular style recommendation letter. Is the Teacher Evaluation form required as well?</p>
<p>The Teacher Eval form is just a form for submitting a teacher rec. It is the preferred form to use for common app schools, but just about all common app schools will accept a rec letter without the form. Just be sure your teacher’s rec letter clearly identifies you. Include your full legal name, HS and common app ID on it, if possible. </p>
<p>It may help your teacher if you print out the teacher eval form for him/her so everything that’s expected is covered in the rec letter. You can get to it by inviting the teacher through your common app account and not including an email address when you do the invitation. That will give you access to a pdf version of the form. </p>
<p>Even if you aren’t a senior this year, you can still create a common app account and get access to the form this way.</p>
<p>In my school all teachers omit the teacher eval form except for courses taught and how long we have known student. We DO include a detailed letter of recom. which better reflects the student. We find the checklist doesn’t truly represent the student. Colleges are fine with this.</p>
