My school does not utilize the online Common Application service for teacher recommendations, so my recommender must submit his evaluation by mail. Is there a form that my teacher needs to fill out to send along with the evaluation, or does solely the evaluation need to be sent?

Dear mdemo7 : Your school is placing your entire class behind the 8-ball. Tell your principal and guidance department that their ignoring the common application process might feel good from a moral position (“we are different and supplying personal service”), but it is costing your class acceptances into those institutions using the common application. With a school like Boston College handling 30,000+ applications, do you think that the ad-coms are looking for more paper processing? Think about the sheer volume (or weight) of paper associated with that many applications.
