I have asked two teachers to give me a rec, my history teacher and my math teacher. My history teacher has requested that I just give him stuff on paper so he can write individual recs for some colleges and send them himself. But that's not the problem.

My math teacher is just going to use the common app but it has made me curious. Do teachers only fill out one recommendation form on the common app for all colleges? How does the common app work for teachers?

For common app recs, you add their email address and they will receive an email with instructions on either submitting online or through the mail.

And yes, there is only one Teacher Evaluation form, and that gets sent to all Common App schools that you assign that teacher to

Are you sure that’s the case? Inviting teachers is a separate step from assigning them to schools. The whole list of the people you invited shows up in one place, but that doesn’t mean they are assigned to all your schools. Go into the school forms section of your common app account for each individual school to see which teachers are assigned to that school. You should still be able to remove and change them if the recs haven’t been started.
