This is a bit confusing, but important. Help please.

I had my name changed a few years ago. (I was Vietnamese, and my parents wanted my Vietnamese name to become my middle name, and I received an American first name) My social security number stayed the same.

I never informed the school of this. I thought it would be too much trouble because all of my past records have been with my old name.

When applying for FA, the social security number is required. The colleges will see my American name, and not the one on my school records.

They might think I'm a totally different person. How would this affect my FA chances?

Also, all of my standardized test scores are with my old name. How can I explain this?

Most likely, you will need to supply them with documentation showing the legal name change. This would include paperwork from the court that authorized the name change. If your name was not changed through the court system, then you would need to visit the Social Security Office and have it changed back to your legal name...or request the name change through the court system.

As long as you have the supporting documentation, it shouldn't be a problem. Schools are used to name changes, especially since women are notorious for changing their names at least once.
