<p>If so, should I include "Insert-Title-Here by Insert-Name-Here" or the traditional name and date in the top right corner followed by a title?</p>

<p>You can, but I don’t think it’s necessary.</p>
<p>1.They already know your name
2.They’ll find out what your essay is about by reading it + The prompt already lets them know what it is specifically about, unless your essay is the “other” option.</p>
