It says on the website that transfers must submit a letter from the dean saying they are in good standing. I was wondering if this just had to be an actual letter, or if there was an actual form that I am just missing somewhere that I need to give to the dean’s office?

there isn't a form. Your dean just has to write a short letter that basically says "I have never had to meet this student on official business" (aka this student is in good disciplinary and academic standing).

and the letter should be on the school's letterhead.
