Transfer into Stanford?

I have seen some forms in here that attack anyone who attempts to transfer into top schools, especially from comunnity colleges, as donors to such school. Please on this forum dont post unless you have real life experience with the transfer process at top universities. I can read Stanford's website and do not need the admit rate or other statistics riddled off.

I am a california community college student.

College GPA: 4.0
Sat 1: 2010
EC's: Founder and CEO of a NonProfit OrganizedVoices (Voter registration and organizing), Public Relations Officer for PTK Honor society, Founder and President of Political Science Club, Going to europe this summer to start a research project on how other countries view USA's Political actions, and planning a massive program next fall to give the local homeless population the Thanksgiving all deserve.

Reason for not entering stanford as freshman: My high school GPA speaks for itself. I was drinking and partying often with no real care for school until before my freshman year of college when i ended up in ER for drug related reasons. Due to a preexisting heart disease (Bicuspid valve and dextrocardia) there were serious fears of reprocutions if my heartrate/blood pressure spiked too vilontly. All ended up well, but the experince scared me clean and got me focusing on life and just how short it can be.

Do my High School grades pretty much destroy my chances?
Do i need to raise my SAT score?
And what could i do to maximize my chances at Stanford?

Don't worry all of my apples are not in Stanford. UCSD is in my residence and so through TAG i should easily gain acceptence and I am also hoping for Berkeley.
