TX CC Student trying to transfer to CNS Math

I am trying to transfer from a community college in Texas to CNS as a Mathematics major for Fall 2016. *2nd choice is COLA majoring in Philosophy.

GPA: 3.64, 4.0 major GPA
Credits: 59

Math courses:
College Algebra for STEM Majors - A
Trigonometry - A
Calc I - A
Calc II - A
Calc III - A
Differential Equations - A

I also took an Introduction to Mathematical Proofs course at a small local university, which I also got an A in.

I am a member of Phi Theta Kappa and Mu Alpha Theta, which is a national mathematics honor society for two-year schools. Also, I have been a tutor at my community college in the Math Outreach Center for 4 semesters now - tutoring everything from developmental math to Differential Equations, also leading study sessions for groups of up to 15 students.

I’m not super confident in my essays, to be honest.

Do I have a chance? Thanks in advance.

Your stats look good to me. Those math grades are really going to help you out. CNS admission criteria aren’t as strenuous (besides compsci, of course) as some of the other schools, and all your major grades are perfect, so you definitely have a pretty good shot, in my humble, humble opinion.
