For my MIT applications, I thought that I emailed both my math and history teachers recommendation forms. However, it turns out that I only sent my math teacher a form, and somehow, my history teacher received a corrupted file. He told me today, and I got him an extra recommendation form, but I turned in my application a few days ago and its already past the Nov 1st deadline.

What will be the consequences of my mistake? :(

As long as your history teacher sends in the recommendation ASAP, I don’t think there will be any lasting consequences. Stuff happens. Not everything is under the control of the student. The admissions committee understands that. If you’re curious, give the MIT admissions office a call. I’m sure they’ve fielded lots of “panic” calls in the past week.

For the rest of the college app process, I suggest that you follow-up on your application components (e.g., rec letters, transcripts, test scores, etc.) in a more timely manner. Forms for rec letters should be sent to teachers several weeks in advance (at least 3 weeks). You should follow up on them at the very latest 1 week before the deadline.

Don’t even sweat about this. This definitely won’t break you. You have to weigh your whole package. If its strong enough this won’t matter but i’m sure everything will work out.
