Your scores are great, and 15 APs looks very impressive!!!
Your ECs and Awards are nice also!

HYPS: good shot (though it's a reach for everyone)
MIT: also a reach for everyone, but definitely worth trying in your case
Cornell/Duke: excellent chances

Good luck =)

Your test scores are really excellent! Your GPA/courseload are also good. That being said, your weakness is probably your EC...they're pretty mediocre and kinda all over the place. Your URM status will definately give you a boost. I think your best shots will be Duke and Cornell, but you definately have a good shot at HYPSM.

In at Duke, Cornell. Really good shots at HYPS, MIT.

Does anyone else have any opinion on my chances at duke, cornell, unc chapel hill (out of state), UChicago or WashU?

I'm also top 5% in a top 100 US News school if that helps at all.

GPA-slightly below to within expected ranges
Rank-unknown (check school profile to see if it has a grade distribution graph)
SAT I-within expected ranges (HPSM, Duke) to excellent (Y (no W component), Cornell, UNC-CH, Chicago, WUSTL)
SAT II-good (680 US Hist saves it from almost maximum possible)
APs- very competitive (high number, 4.44 average)
Awards-within expected ranges
ECs-average, but very scattered = C+ class, because you did exactly three activities for more than a year, and none for more than two
Work-good, but unable to hold a job for more than 12 months
Hooks-URM
Thus I must say, with your relative lack of commitment:
HYPSM: Crapshoot
Cornell: Good
Duke: Above Average
UNC-CH: Very Good
Chicago: Good
WashU: Above Average

Well, I've also been a member of ATHS all four years, a member of SNHS (Science National Honors Soc.) for 3 years and Science Club for 3 years. My first job was this past summer, so > 12 mos is impossible, and I'm still at my current job. I'm doing the math/programming competitions again this year and I'm currently taking Multivariable Calculus/Differential equations. And whether or not it helps, I'm getting all As for the midyear report.

Work-good, but unable to hold a job for more than 12 months

LOL, this dude is lazy! You sound like the unemployment agency. (i'm being facetious). Good chance at all those schools, only (very slight) weakness is the third SAT II, but only harvard schmarvard (and yale, princeton) needs 3 of 'em.
