Vaden Health Forms?

<p>On the Approaching Stanford Website, it says that we had to fill out the Vaden Health forms by June 30. However, there are no forms on the Vaden website and there are no forms in the packet they mailed us. Does anyone know what we're supposed to do?</p>

<p>The forms are online...just follow the link from the approching stanford website. Then, under Submission, click on "complete your online medical requirements form." You have to then enter your SUnet ID to access the forms.</p>

<p>I have been overseas and thus have trouble accessing the Vaden forms online. I called Vaden and was told that I would be put on an academic hold should I come to Stanford without completing the entrance requirements. Can anyone who's done the forms let me know what's in there? What vaccinations and records I need to do and provide? I will come back to the U.S. before orientation so that I will still have time to complete the forms if they are all about checking some boxes about my health history. But if there are any vaccinations I need to get, I hope to do them before I start school.</p>

<p>Did you get the Approaching Stanford mailing? It tells you all the vaccinations that you need. You could access it online at approaching.stanford.edu where you can also find the link to the vaden health center forms.</p>

<p>you're on academic hold anyway when you first get to stanford -- you don't get off of it until you meet with your advisor. i got some vaccinations when i got to school too.. i didn't have meningitis so i got it in about october last year</p>

<p>if you plan to waive your coverage, make ABSOLUTELY sure that it axcess says you've waived it. otherwise, the vaden folks can be quite about unfriendly about accommodating you later on (as I learned the hard way)</p>
