<p>My friend said something about a form for Vaden. Is it too late to turn it in/where can I get it? I vaguely remember something about it, but can't find my paper =(</p>
<p>I think it was a medical history form, if I am remembering correctly. I can remember exactly what it looked like, but I'm a little hazy on what it was for. Rest assured though, if you messed up or forgot something iimportant t will appear in one of two ways: either as a charge on your university bill, or as a hold on your enrollment on Axess. If you see one of those things appear when they shouldn't, contact whoever seems appropriate, and either rectify the problem if it's a hold on your enrollement or claim stupidty if its a charge on your bill, and hopefully you can get everything sorted out.</p>
<p>Yeah there was something like that last year too. I think you can do it online or something. Just call 'em up and talk to Vaden its not like they're going to cancel your acceptance because of it ;)</p>
