What do these abbreviations mean on the bottom of NMS application form?

<p>My son submitted his finalist application form today. On the bottom of the second page it has these abbreviations and a number associated with each of them..</p>

<p>The last three are major, career path and college choice #, but does anyone know what the first 4 mean?</p>

<p>I was wondering about that too. My daughter’s rank says 4 and her class placement is in the top 4% of her class but I’m not sure if that is just a coincidence. I know that many high schools don’t rank anymore. I don’t know what cl, %col and type means</p>
