What if one changes one's high school...

Hello everyone.
I am a high school senior. I will be applying to some universities, majorly US, this year.
I just want to know as to what happens when one changes one's high school. I shifted to New Delhi to complete the last two years of my high school.
As I am needed to submit school transcripts of grades 9 to 12, I am really confused on how to get a single transcript. Should I get my current scores of grades 11 and 12 mid-term attested with a copy of score sheets of grades 9 and 10?
For those who know the Indian education system: Will it be a good idea to simply attach the CBSE Grade 10 score sheet with my current scores?
Is it necessary to have your counselor write about your activities at that school only? Mentioning grades 11 and 12 only will lessen my chances as I did a significant amount of work before joining this new school. And there can be only one counselor writing about you, right?

Thank you for your time. Cheers!

What most often happens is that the counselor at your old school fills out the Secondary School Report based upon your time there, and forwards it (and the transcript if your new school does not already have) to your current counselor, who appends it to his/her recommendation before sending to the college(s).
