I've got extracurriculars and work experience as the main body, but I'm more concerned about the header....name? name and gpa? name and date of birth?

any thoughts?

name and social security number

So just ID-related stuff, then?

Should the "resume" just have activities on it? My daughter divided it up per year and listed her ec's and summer activities. They just fit on one page. Does she need to add awards and honors to this in order to make it a more complete resume? If she does, it would be longer than one page.
"Awards and honors" is listed as a separate question on the common application, so we thought it does not need to be included on this "resume".

Can we add a separate sheet for awards and honors too?

Thanks

I don't know anything about it, really, but here's what I'm thinking:

Awards/honors on the common app seems to apply to general awards (national merit, etc) that aren't affiliated w/ a particular activity. with that in mind, i divided my ec's (summer too) up into categories, listing awards and, in some cases, mini-descriptions, next to each ec.
