will my admission to Cal Poly Pomona be rescinded?

I'm a provisional admit currently which means i've been accepted and all they need is my final HS transcript but i fear i might get a D in Calculus my last semester of senior year. their admission requirements say this:
Applicants must complete the following 15-unit comprehensive pattern of college preparatory subject
requirements with a "C" or better:

English, 4 years
Mathematics, 3 years: algebra, geometry and intermediate algebra (all students are encouraged to continue taking mathematics through the high school senior year)
Social Science, 2 years including U.S. history or U.S. history and government
Science, 2 years with a laboratory (one biological and one physical)
Foreign Language, 2 years of the same language (subject to waiver for applicants demonstrating equivalent competence)
Visual and Performing Arts, 1 year: art, dance, theatre/drama, or music
Elective, 1 year selected from any of the above subject areas.

I've passed all of these classes with a C or higher however, it also says this:
• Complete all courses in your senior year with grades of "C" or higher

Seeing as I already passed the 3 years of math (algebra, geometry, algebra 2/trig) required with a C or higher, will this D grade in Calculus get my admission rescinded? If it can, how can I contact them to explain why i got the D? (my parents separated and my dad and I lost our house to foreclosure. the emotional stress distracted me from my work).
Note: I also got a B my first semester in Calculus if that makes any difference.
Thanks
