<p>I was going through the health form and I am lost. I am an international student and in my country immunization requirements are different from US requirements. I noticed that I have to get MMR shots. So i got my first dose a few days ago and now I can't enter the date of the second dose since it is in the future!!!</p>
<p>And also it seems like some of the vaccines are not given in my country and there are alternate vaccines for them. So can I just submit the form with blank fields and write the details on the printed form and post it? The form says that I can't return to the form after I submitted it. -.-</p>
<p>If you have any concerns or questions, call the student health center. They can put you through to a nurse who will be best at answering your questions. They were very helpful when I called. </p>

<p>Hi, sorry I’m late about this, but can we send them the health history form before we take a required immunization? Or can we just mail the form and update them later by faxing the docs?</p>
<p>We will be sending my son’s prior to one of his immunizations, because he needs five years between meningitis shots. That means we need to wait another month for that shot. We will send the form in with pending on that shot. Call Gannett if anything is unclear. Link is above. He just got home from doing some blood work. That may or may not come back prior to the due date for these forms. Again, we can write pending and send the labwork later. Just get your physical done and get the forms sent.</p>
<p>Thanks people. I called Gannett and they told me to send the form with some shots pending and follow up when I actually get the shots. I just posted the health form today. Good riddance :D</p>
