So this is what was received. We've sent the BC and it has been returned.

I didn't get to see that part. Oh boy. How does one get a notarized photocopy?

I've heard from some people, that they received a confirmation of acceptance..i have yet to get that...As for filling out paperwork--I think the next thing we should be expecting is the permit to report package, which from what i've heard will come around May..i think
There's a copy of an old one online http://www.usna.edu/Admissions/documents/permittoreport07.pdf

Was the request for proof of citizenship paper thing, part of the first package of papers that came along with the appointment?

"They need their birth certificate? A copy?"

Deja Vu....................

Same piece was missing from our kiddos Appointment package NP.

The ever-resourceful, very kind, WP to the rescue!

He sent an original BC last week, got delivery confirmatiom Tues., yet to have the original returned.

P.S. Kiddo enclosed a SASE for return.

nurseypoo, i think you can go to the bank and have a notarized copy of whatever you need, say for instance a copy of your birth certificate.

S had to have a notarized copy of birth certificate for a trip to Mexico 2 years ago. I got it at the records office of our county government offices. It cost $10.

I didn't receive dental record in my packet! but i have sent in everything else, notarized copy of my passport, acceptance form, and background check.
Am i good to go, or do i have to expect more paperwork to be given to me?
thanks

All candidates need to send in a panorex dental x-ray.

All candidates need to send in a panorex dental x-ray.

Our Mid got the panorex and her dentist (ex Navy dentist) told her, now is the time to get your wisdom teeth out. Not Plebe Summer!!

His BC is from birth abroad. Hmmm. I get kind of nervous thinking about mailing that off.

Wisdom teeth appt. on 21st. Will get panorex, then.

Thanks a BUNCH WP!

Wait, where does it say i need to send in dental record from my first package?
i don't recall reading that

It was on its own sheet in that white envelope that was enclosed with the appointment.

It seems some info was absent from some Appointment packages. Glad we have each other for back-up! WP was my lifesaver on the missing piece of the puzzle :)

NP- I suggest sending it Certified mail if you have to send the original. His original came in today's mail, And they didn't use the SASE he included

Our PTB <plebe-to-be> had his wisdom teeth out over the Christmas holiday. Since the most recent Panorex <fall '06=""> revealed the impacted wisdom teeth, I requested a operative/discharge report from the oral surgeon which went along. That satisfied the dental branch.</fall></plebe-to-be>

SASE?
haha

I never received any information about any dental work in my appointment packet!?!?

this is crazY!! all you need for the Dental exam is a recent Panorex (recent as in no more than a year old). if you need the address or want me to scan that letter for you, PM me!
