Applying for reinstatement, questions about the essay

I’d like to appeal for reinstatement at my university and I have a few questions about the essay. These are the requirements.

1)Analysis of factors that impeded academic success during last enrollment
2)Description of steps taken to overcome this
3)Details of studies undertaken during absence, Statement of academic plans in my absence (where enrolled, grades)
4)Statement of plans in future, major, type of academic program to attempt 1st quarter back

My background
Last attended March 2016
Worked for about a year, then started at a community college, Fall 2017
Have bipolar disorder and anxiety, I’m now taking different medications and attending therapy, this seems to be working

Questions:
Ideal length? One page maximum?

I have withdrawn from school before. Should I mention that in the essay?

Should I mention that I have bipolar disorder? Or should I just say depression, or episodes of depression

I was thinking I should not mention that I'm taking medication, and just talk about how therapy helped me break the 
patterns that lead to not studying and bad grades, and pretend this alone managed my mental health problems. Or would 
that be a mistake?
