Arts supplement question

<p>I’m sending an arts supplement, and I need a recommendation from my teacher. Which form do I give her though? Do I just give her the teacher evaluation form from the Common App? That’s what I’m guessing since the Amherst website/app doesn’t specify, but I just wanted to make sure…</p>
