<p>When I filled out my application, I included that moving to different schools in the midst of my freshman year had caused me some slight anxiety and depression issues, resulting in a slight decrease in my academic performance (it isn't a large impact - my grades appear fairly consistent throughout the years). They later sent me a questionnaire that read: "In your application you disclosed a physical or mental impairment, or medical condition. As part of our holistic review, would you like us to consider the physical or mental impairment, or medical condition?"
While I did experience some anxiety and depression, it was not so serious that it required medical attention, etc. So by calling this a "mental impairment or medical condition", I feel like it's rather a stretch - I mean, aren't negative feelings & stress inherently associated with moving. I'm afraid to submit this questionnaire because I'm not sure if I'll come off as whiny and exaggerating. Should I submit it?</p>
<p>If it hurts your pride don’t submit it. However, it can only help.
For reference though I’m legally blind and didn’t get said questionnaire…</p>

<p>Also, I’ve heard that they’re still sending some out. As long as you listed it in your app, you should receive one.</p>
<p>If a doctor didn’t diagnose you with anxiety or depression, you can’t claim the disability.</p>
<p>@MaineLonghorn: I even sent them an email saying that I had never been diagnosed or visited the doctor for my “disability” and they simply responded that it was my choice to submit or not and that it wouldn’t affect me negatively. </p>
<p>So I guess it doesn’t need to be an “official diagnosis”.</p>
<p>I talked about illness and I didn’t get a disability questionnaire at all. Though all people who mention disability/illness get the form.</p>
