<p>I'm trying to sign up for Bruin Direct and they said I have to put all the forms in the yellow return envelope. However, I did not see this included; does anyone know what address I should mail it to?</p>

<p>The address is on the form (2nd paragraph). I don't think they ever send the yellow envelope. Whoever is stuffing it forgot or they ran out of envelopes.</p>
<p>when you waive SHIP it gives you option to waive it for specific quarter I did it for them all</p>
<p>Just be sure that if and when you waive SHIP that you get a confirmation number, waive the entire school year AND be sure to waive before the deadline. </p>
<p>I've worked in the SHIP insurance office and soo many students every quarter screw these things up and end up paying for their private and SHIP insurance.</p>
<p>Actually, you should get three confirmation numbers, each corresponding with the three quarters.</p>

<p>You should call your insurance provider. That's what I did when I didn't know if I met any of the requirements. It may just happen to be UCLA Medical. :)</p>

<p>if you have private insurance and it meets the criteria set by UCLA you can waive SHIP, which is student insurance provided by UCLA. You save like $600 for the whole year</p>

<p>you can waive it only if it meets specific requirements. if you have an hmo, you have to find a new doctor in the area.</p>
