Okay - so your worst case scenario per the NPC (which isn’t a guarantee but hopefully pretty accurate) is that you’d have $2850 expected indirect costs, plus the maximum in guaranteed loans (which is $5500 the first year but will go up in subsequent years - $27K total over 4 years) and $2500/year in work-study. I think the likelihood is that any merit you get would be applied to reducing/eliminating loans and work-study. I think it’s unlikely that they’ll give you additional cash-out toward those projected incidentals (though I’d be happy to be wrong), but you could still elect to take the guaranteed loans to cover that.

The other downside of a BS/MD program (assuming that even becomes an option) is that it many of them reduce the number of fully-funded undergraduate years you would get, while they may or may not reduce the number of debt-incurring med school years. (Looks like CWRU’s is an 8 year program, not accelerated, so you’d still get your 4 undergrad years in that case… but as others have noted, it’s a very long shot, a reach even for those with perfect stats.)

Even if you get a 1500 on the SAT, PPSP will be a reach, and a high one. @Bopper linked to some good info about that program, I expect it has a single digit acceptance rate.

From my interactions with admissions, about:
1700 apply
60-80 are finalists (offered an interview)
20-25 are accepted into the PPSP program
