Band for 4 years
A few other now insignificant extracurriculars

GPA: 3.94
UM GPA: 4.00
MTA and MACROE endorsements
Total CC credits at time of application: 66 (42 transferable)
Total CC credits at time of graduation: 84 (50 transferable)
Expected degrees by May 2017: AAS in Social Work/Mental Health, Associate in Arts, Associate in General Studies
Three part-time jobs, including one with local school district driving busses - youngest employed by 14 years
400+ hours of Internship with youth residential mental health facility
Three good to great letters of recommendation

FWIW, I applied as a freshman in 2013 and applied as a transfer for fall 2016 and was rejected both times, however the transfer application was rejected on a pretty personal basis. I’ve also been in contact with the admissions department.

Bonus Question!! The admissions department counselor I spoke to told me to send any information to add to my application to her directly. I plan on writing a letter detailing my experiences since last applying. Should I include a willingness to take summer classes, either at the community college or at UM to catch up to junior standing of 55-60 transferable? Or am I close enough at 50 to not bother?

Double Bonus Question!!! What other schools should I apply to as a CC stand out student? Looking to major in Psychology with either a second major or minor (at UM) in social work that would allow me to get a MSW in an accelerated program, to move quickly onto achieving a masters or PhD somewhere in the realm of Psych (probably Clinical or Educational). From safety to reach, any input is welcomed.

You need to check with the transfer office about which courses transfer. Courses without a direct UM analog will not. I think you have an excellent shot but it is always hard to say at UM. I’d use MSU as your backup. Wait a year to see how your year goes at UM, by then you should have a better idea of which grad schools will work for you. You’ll have to time to explore programs and speak with faculty have attended there. You may also decide that an MSW is not for you (poor ROI). Often times a favorable recommendation letter from a professor to their alma matter makes a huge difference in grad school admissions - they are not handled the way undergrad admissions are through an admissions office- departments interview and select their Ph.D. candidates.

New ACT/SAT scores would be advisable.
