Stellar chances because of your test scores combined with your URM status. Just make sure your GC explains any dip in your GPA.

Great SAT and ACT scores, amazing level of extracurriculars. I would recommend discussing your involvement in these ECs in your essays- maybe pick one that means the most to you and discuss how it shaped you as a person. Your GPA can be explained by discussing your periods of hospitalization. Overall, I think you have a very good chance at UC Berkeley, UCLA, and USC. As for Stanford, your test scores and activities definitely qualify you as a match. However, it’s important that your essays shine, and your teacher recommendations are amazing. They can read the list of your achievements, but they want to hear how these experiences have affected you.

@FiveFactorial, congratulations on your fine achievements. The schools you have listed are never sure things, but you are extremely qualified, and your hooks give you an edge. I would add surviving cancer and overcoming that adversity as another factor, and something to potentially discuss in essays. Your 3.8 GPA should not be a major factor given that context. Your qualifications are all the more impressive given what you have had to overcome. Make sure that your essays are strong and personal - don’t be superficial.
