Confused about school forms

Hi! I am currently filling out my applications for Georgetown and other colleges on the Common App. I am a little confused with the school forms. For one of them (the School Report), do I have to physically meet up with my high school counselor again and have him fill out, sign, attach transcript, and mail the form to the respective colleges? Or can I just attach the high school transcript on my own and mail it?
Or can a college professor fill out the School Report? Things are just so hectic and confusing to me right now.

One more question, must the school forms be mailed and received by the colleges by the respective deadlines, or just postmarked and sent before that deadline? Some of my professors and counselors take QUITE A BIT to fill out and send materials.

The Secondary School Final Report form must be filled out by your HS GC, not a college prof (they fill out the Instructor Evaluation form). But you don’t have to physically meet with them.

First fill out the top of the form and sign. Then give/send it to your HS GC to fill out the rest, make copies and sign. They should put the form in an envelope, seal and sign over the seal. The form asks that they transcript be attached, so if possible, arrange for your GC to do that, but if not, you should be able to send the transcript in a separate letter. You will want to either provide your HS GC with the stamped, address envelopes to send directly to schools, or have them return them to you to mail.

You need to check the websites of the colleges for deadline information. MOST just want it postmarked by the deadline, but I did run across 1 school that wanted everything received by the deadline, so it can happen.
