My school uses one counselors form and sends it out to every school. It’s called the Secondary School Report Form. Basically, it’s got a top for the student names, address, etc, part 2 has the school information, GPA, Senior year classes, etc, and part 3 is where the counselor writes a letter of recommendation. Do any of your schools use a similar system?
I asked my school, and they said that for the schools that say I need to print out their counselor form, this form is acceptable.
