I filled out the online midyear report way before the 2/15 deadline, but I didn't rush to send it in, i wanted to hold it off till the last minute to see if i can win a medal at the ISEF contest. Well, as 2/15 approached, I had completely forgotten that my midyear report has yet to be sent, although i did remember to give in my counselor's midyear report (which chicago received on 2/23).

Yesterday, 11 days after the deadline, i suddenly realized my big mistake, and hastily sent in the report. how badly will this kill my chances for admission? should i write a letter to the committee and apologize/explain for the lateness?

no, im pretty sure your ok. They mostly reviewed the other students files which are complete, and will eventually review yours once they get the appropriate information

I vaguely remember handing my counselor half a page of something concerning the mid-year report. However, I'll agree with Esquared because that's what the website said.

I gave my guidance counselor the half-sheet that I printed off from the Chicago website. The only required midyear report is the online form with self-reported grades and all... If you haven't sent it yet, do it quickly! And yes, a letter of apology/I love Chicago! wouldn't hurt either.
