Does anyone have last year's teacher evaluation form? (In pdf)

<p>Did anyone save the file? I saw the evaluation form before but I didn't save it to my computer, it's something like a two pager, there are questions like "Does the student ask questions?", the answers are to check boxes, they are [good, ..., best in career.] type. It says 2006 on the upper right hand corner. </p>

<p>I am trying to apply in 2007, however, I want to show the form to my teachers when asking for recs, to just give them a feel for what it is.</p>

<p>hey guy, calm down a little bit. don't apply to college six months early. some parts of the application (like the recs) will change significantly next year. why not wait and do things when everyone else does them, and, in particular, when the forms are released?</p>

<p>i promise we don't give bonus points for submitting the application in mid-july.</p>

<p>LOL, I am not going to submit it early, it's just that later on I would be busier than I am right now. About the point of "will change significantly", what kind of change are you talking about?</p>

<p>ok recs don't take much time have a resume ready and give it to the teacher with the other forms and make sure you give it to them by 2 WEEKS (not 6 months) before they are due. Then you are done. Recs are simple and really you have plenty of time to do the app just chill enjoy the summer next year will probably bring lots of apps just relax and let the summer roll.</p>

<p>Yes, that is a bad way to do it. The standard procedure is to give your teachers the recommendation, a stamped and addressed envelope (to Caltech admissions), and ask them to mail it (via the school office, usually) when they are done with it. Giving you the envelope, even sealed, would make it possible for you to read the rec... it will look better for you if you let them mail it.</p>

<p>As for "what kind of changes", wasn't this the whole point of my previous message? Calm down and stop trying to figure out the application before it's released. I promise the new form will be available in time for your teachers to fill it out. If you try to give them old forms or predict the new one, you will waste their time and yours, which could be better used learning Galois theory and picking up girls.</p>
