I had contacted admissions about that exact question and they said just send it with everything labeled with:

Your full name

Your School

Your Date of Birth

They said they didn’t require any kind of labeling forms or the like. Good Luck!

Hi i want to send a book but it still hasn’t been published. Is it ok if supplementary materials are sent after Jan 1??
@Arch54, technically, Jan 6 is the deadline for arts supplements. If it’s in before then it will be fine. If it isn’t, I would contact the admissions office and ask them if it’s acceptable to send it in past that date
@Arch54 Don’t send the book. No one will read it and it comes off as pretentious. It’ll end up in the recycling bin; I guarantee it.
Hi T26E4 thank you for your reply, I don’t really want them to read it, its more like proof that I have written the book. And could you please tell me why it would come off as pretentious?
@201698 Thank you for replying but does a math book come under the category of arts supplement?
A math book is not an arts supplement. If your book is self-published, then I would not send it in or mention it again (I assume you put if somewhere in your application). If it is published by a reputable publisher, then I would send a short email to the university stating that your book has now come out, give a link to the Amazon listing or whatever info you have about it, and ask if they can add that info to your files.
@midatlmom that’s the problem though, I am waiting for Amazon’s approval and that takes about two days which is why I thought I should send it to them instead
@Arch54 “If it is published by a reputable publisher…” not a self published book that gets an Amazon listing. That’s no indication of quality whatsoever. My kiddo’s 10th grade IB class all self published books. Your book has NOT been published by a reputable publisher who has invested in it, expecting it to be a commercial success.
@T26E4 it is being published by a publisher whose math books were a best seller here in Singapore. I think he does believe it to be a commercial success because the math teachers who reviewed it felt it was a pretty good book.
It is not self-published just to clarify. Another interested party were willing to invest in order to get the book published
@Arch54, I suppose that’s not an arts supplement after all. Regardless, it shouldn’t matter if it’s late as long as it’s in within a matter of days. And to those who are telling you not to include it, that’s ridiculous. If it speaks to who you are as an applicant, if it’s something you’re proud of, if it adds to your personal narrative… Include it! Send it in! Best of luck to you.
@201698 , Thank you! I was starting to think everyone would just say no. It does speak a lot about me as even my common app is about Math. Thank you once again
