Enrollment Verification of Family Members Form

<p>Last week I got this email:</p>

Form for each family member that will be attending college. </p>

<p>A personalized copy of the form is available on My Aid at UCIrvine</a> Office of Financial Aid and Scholarships Home .<br>
Log in and go to "Print My Forms" to obtain your copy.<br>
This form is due in our office no later than October 1, 2009. Failure to return
this form by the deadline date will result in the following:</p>

<ol>
<li> Cancellation of winter and spring financial aid awards.</li>
<li> Re-calculation of your financial aid eligibility.</li>
<li> Possible retroactive billing of the aid you received for fall quarter.</li>
</ol>

<p>A separate form is required for each family member enrolled in college, not
including yourself. If more than one verification form is required, you should
print the required number of forms.</p>

<p>Please be aware that your Winter 2010 aid is on hold and you will not be able to
access your aid to pay your fees.</p>

<p>So inititally i filled it out for my brother who is going to college and I faxed it in. The thing is it has been a week since I faxed it and it has not been update on my financial aid file. So now I'm worried because it says "more than one family member attending college during the 2009-10 academic year" in the first line of the email. What does that mean? Only my brother attends college. My mom is taking one class at a local college but that doesn't count (and I didn't put that down in the fasfa either since it's less than part-time and they don't include parents).</p>

<p>So did I do it correctly and they just haven't updated it yet? Or did I do it wrong?</p>

<p>BTW I tried calling all day long the past week. Can't get through. </p>

<p>they probably just didn’t update it yet. the deadline is october 1st. when you get to uci, just make a visit to the office and see if everything is ok. that way you have time to fix it if it you need. but they are really busy and prob just haven’t updated yet. i’m in the same situation as you, and they haven’t updated mine yet either. and yeah…they don’t answer the phone anymore.</p>

Number of Enrollment Verification of Other Family Member Forms Needed:1
Number of Forms Received:0</p>

<p>so does that mean they only need my brother? because and this point i’m kinda wondering if i actually did put my mom down on the fasfa thing…i mean if i did it would say 2 correct?</p>

<p>To be safe just mail in another verification for your brother - it only says they need one, so it has to be for your brother. Then relax.</p>

<p>i faxed in my paper on tuesday and it isn’t updated yet so i will probably go in on monday once i am moved in :/</p>

<p>i went in today about mine. they said they have a receipt that they received my sibling’s fax. they just haven’t updated my file yet. it’s prob the same with you. and i faxed mine in last friday. they said they’ve been really busy and have been getting a lot of faxes in.</p>
