<p>How should I format my supplemental recommendations... write my name, SS number and school on a sheet of paper and have them attach their letter?</p>

<p>I would make a copy of the regular recommendation form, but write in large letters at the top that it is a supplemental recommendation. That way there is no confusion and the college will have the recommendation and identifying information they need.</p>
<p>This is the way my daughter did it anyway.</p>
<p>Some colleges specifically tell you the identifying information they want on supplemental information (which may be SSN, but I've also seen address and birthdate used). But if you use their form, it obviously gives them the information they need to put something in the right file.</p>
