<p>on the website they have the final transcript request form but i got in the mail a proof of graduation request form. they ask for the same stuff. does it matter which one we send? or do we have to send both?</p>

<p>There's a form that your counselor fills out, along with a copy of your end-of-year transcript.</p>
<p>yeah. i don't which form i'm supposed to give my counselor. the final transcript request form, or the proof of graduation form. if you look at them, they are almost exactly the same.</p>
<p>They def want your final transcript. I've never heard of a school not wanting it and they seem to be even more hardcore about it at Northwestern.</p>
