Financial Aid Appeal: Evidence of Mental Illness

I don't want to go into too much detail but I was diagnose with a certain mental illness that cause me to feel depress and unable to concentrate, this was weeks after I start my college . However, pills take several months to work and I have to withdraw a few classes which lead to my suspension. I read the appeal letter and I come to this.

"This paragraph, explain how did you get into this situation. Be detailed, but also concise. Please note that this is a place to indicate if you have evidence that
can be attached for support".

I'm not sure what I should ask my psychiatrist for as evidence. My mental record? Letters? Sticker?

Thank you.

Your physician can write a letter in support of your petition, indicating that you suffered from a health condition that is now under control.

In general, a threat of suspension usually has some steps- did any Dean request to speak with you? And, have you been in touch with the disabilities office or health clinic? FA is only one piece of this.

Well,I don’t know if suspension is the right word, but they said I have to do an appeal paper. The place I go to is a health clinic, I’m just not sure what should I ask for.

what you described was depression and ADHD.

Ask your college for more specific details about what your physician would need to include. And ask your physician about his/her experience with this kind of letter. Someone at your clinic has written similar letters in the past either and will know how to do this.
