<p>That should do it. They’ll be sending their letters directly to the admissions office. You just want to make sure they put enough information about you on it that it goes in the right folder.</p>

<p>^I believe that should be sufficient. If someone does share your same name, you can guarantee (for all intents and purposes) that he/she will not share the same date of birth.</p>
