Forms?

<p>I got a mail saying I need these things before I start classes...</p>

<p>*Proof of Mandatory Immunizations for all incoming freshmen and transfer
students.</p>

<p>*Authorization for Treatment of a Minor for any students who have not yet
reached their 18^th birthday.</p>

<p>*Authorization for Release of Health Information must be completed by any
student who wishes to give parents/legal guardians access to their
health information.</p>

<p>I might/might not have these forms, but were they sent in the mail some time during the summer? If I don't have it, is there somewhere I can print it from?</p>

<p>Actually, never mind, I found the mail…but gg, I forgot to do the immunization thing…Any way I can get my parents to get the immunization thing here at my doctor’s place then for me to make a copy of it?</p>

<p>If you have a copy of your immunization record then just send that in.</p>

<p>Yes but if you have all your shots you don’t need to go through the trouble of going to your doctor and filling that out. Just make a copy of your immunization record and send it in and you’ll be done.</p>
