Governor's Scholarship

<p>I am about to withdraw the money from the governor's scholarship I received from high school. I believe I am supposed to fill out this form and send it to Boston:</p>

<p>I am confused about filling out section 2. </p>

<p>Who should I make the check payable to? (UC Regents?)</p>

<p>What should I fill in "attention?" Financial Aid Office?</p>

<p>And school mailing address?</p>

<p>Oh well. Can anyone still give some general advice about this scholarship withdrawal?</p>
