Has there ever been a likely letter rejection or mistake?

I was just wondering. Has anyone gotten a likely letter by mistake? Or been rejected after receiving one without any significant drop?

It’s much easier to make mistakes when you are processing tens of thousands of notifications with different content at in a very short period of time than when you are sending a few dozen e-mails to selected people.
