Immunization Records for UF

Hello all! I have a very important question. I would like to know if it’s possible to just submit an immunization form that was provided by my healthcare doctor? I currently live in Gainesville and my doctor is in South Florida. Because of exams, I am unable to make a trip back to south Florida and ask my doctor to fill out UF’s form. I just saw on UF’s website that they have their own form for the immunization papers. I know that the one I have right now says that I received all the necessary vaccines. Could this be a problem?

I have the same scenario with my son. I emailed health compliance and this was the response.

“Thank you for your email. Generally in these cases, our students have success bringing the old records and the blank UF form to their local health department, a CVS Minute clinic or an urgent care clinic. They will generally copy over the necessary data, sign and stamp the form. Please let us know if you have any further questions.”
