Help about 2004 Tax Return 1040 (pls reply asap)

<p>I'm an EOP applicant.</p>

<p>When college ask to make copies of Federal income Tax Return Form 1040 for 2004. and said in bold Tax return must be signed....................do I send the whole STACK of it (this includes Captain gains and losses,supplemental income and loss, etc), or just the first two pages.......where the second page have the signatures.....cuz im looking at the bottom right corner of the pages and only the first two pages say "Form 1040(2004)" ......are they referring to that?......or including the other pages that have something like "Schedule D (Form 1040) 2004"</p>

<p>Your complete 1040 includes all schedules and forms.</p>

<p>how about the Form IT-201 that's attached to my 1040? do i copy that for them? it's the Resident Income Tax Return......but it's IT-201...(sorry if it's annoying)</p>

<p>That's probably a State form and thus not required if they are asking for your entire Federal return. As a matter of fact, if they didn't ask for your state return, then there's probably a few more pages you don't need to send.</p>

<p>for mine.......only federal is 6pages.....in total....but including the state, it's 13</p>
