<p>And yeah it’s kind of late to do it. The shot takes about 6 months to complete since it consists of three separated installments. But better start it as soon as possible rather than wait.
Also if you’re 19 or older by the time of enrollment then you don’t need proof of a hep. B shot…the requirement is only for people 18 and younger by the time of enrollment.</p>

Click on On-Line Services, then click on Hepatitis B Vaccination Requirement, you’ll be prompted to logon, then you can proceed to fill out the questionnaire and stuff.</p>
<p>When they ask will you be 19 years or older prior to your date of enrollment, when exactly do they mean by date of enrollment ? is it the first day of class ? My birthday is in august and i am turning 19 so im not sure…</p>
<p>AS long as your 19 before September 18 (which is when you have to submit final Hepatitus B info) then you’ll be fine and you don’t need to take the shot series (at least not right away). If you’re going in a health related field (working in a hospital, etc) then its good to get the Hep B shot as soon as you can though.</p>

<p>^ I think you do, I think you have to send that in by September 18 (immunization records or proof of immunity) and they will probably tell you to do that in the coming weeks.</p>

<p>Email them if you have trouble filling out the questionnaire. I have an immunization record in Chinese, so I scanned and emailed it to them and they responded with a confirmation code.</p>
<p>No, you don’t need to send in any immunization records. I already did it so I know. Just fill out the questionaire where you’ll be asked to provide the dates you had your shots, and other info. After you submit the questionaire, you will get a confirmation email from the Wellness center that says the info will be forwarded to UCLA, and also that you don’t need to send in your immunization chart. Save that email.</p>
