I'm using the Common App, so do I have to give him one of those Teacher Evaluation forms and just fill it out to the best of his ability? And how do I list him online since the only options are for counselors and teachers? Please help.</p>

<p>No, the teacher evaluation forms need to be filled out by a teacher. You can use the superintendent as a supplementary recommendation, mail it directly to the colleges. The Common App has no place for non-teacher/non-counselors recs. So you will still need to have 2 teachers and your GC do the evaluations.</p>

<p>No, he is not a counselor. No need to list him on the Common App, as you will have to have your school guidance counselor and 2 teachers listed , with them filling out their forms. The superintendent would be a supplemental rec.</p>

<p>Finally, should I just call the admissions office and give them a heads-up that a supplemental rec is coming in? Should I have the superintendent identify me in any way on his rec?</p>
<p>Yes, be sure the super identifies you by full name, address, maybe date of birth or social security # if you are using that on your application, so the admissions office can add it to your file. It wouldn’t hurt to send the adcoms an email telling them the rec is coming.</p>
