<p>I would recommend providing her with stamped and pre-addressed envelopes for every university to which you will be applying.</p>
<p>Would they also have to fill out a “teacher evaluation form” from the common app site? Because that’s what the common app site says…but in some cases of volunteering, those questiosn don’t really apply at all.</p>

<p>yes, it is OK to submit a supplemental recommendation that does not use the standard form, although some schools do discourage this. You can do this by snail mail as mifune suggests. </p>
<p>As with everything sent by snail mail, be sure the school can match the recommendation to your application by having the recommender include indentifying info such as your full name, high school, birthdate, possibly even your home address. If you aren’t confident about the recommender doing all that, you could ask the recommender to hand you the sealed/signed envelope containing the recommendation and, without opening it, you then could put it in a larger envelope with a brief transmittal letter of your own to the school.</p>
