<p>I suppose it depends on which school(s) you're doing this for and what your situation is. For me, I'm a homeschooler applying to Stanford, and they let you (want you to!) submit extra stuff, including recs. Since I attend a community college for math and science, I gave my "supplemental rec" teachers printouts of the regular forms, but made a note on the stamped envelope that it was an extra rec (also made a notation that I'm a homeschooler -- don't know if that changes anything). When I send in my app, I'm also going to make a note of who the "official" rec writers are and who the supplemental writers are, just so no one in admissions gets confused.</p>
<p>I guess this works fine for teachers, but it probably wouldn't work so well for someone who hasn't taught you. In that case, I would type up on a separate sheet of paper the application's questions for teachers to answer, but only the ones that apply to you/your writer's situation (i.e., you would probably include "motivation" but not "contribution to your class"). Then your writer can just send a letter to the college (you'll also want to make a note on the envelope or somehting that it's supplemental).</p>
