I don't know where I could get in...

My high school grades were horrible. I graduated with a 2.0 UW gpa.

I have now taken my education much more seriously and completed 50 credits at a community college over 1.5 years and have a 3.72 with a 4.0 in my majors classes (political science).

Male and URM

Never took the SAT

I am part of the Phi Theta Kappa honor society and Deans List

Multiple extracurriculars that involve my major since high school

A solid recommendation

What types of schools could I transfer into with these kind of stats?

My dream is either Cornell, Brown, or Johns Hopkins
But I'm also applying to GWU, USC, Emory, and American University

I would like to transfer to a top 30 college but I am unsure if I have a chance since my high school grades are vastly different than my college and it's hard to find acceptance/denials from others on here who are in similar situations.

Any input on if you think I have a solid chance or not is appreciated...

I think that some states have the option of an easy transfer if you meet their grade requirements to your state flagship university. Depending on where you live this can give you access to some fantastic, highly ranked schools. (Ex: UVA UCB, UMAA etc.)
Hope it helped

Look for the Common Data Set for each school, section D. For instance, [Common Data Set](Common Data Set - Institutional Research & Planning) you’ll find Cornell needs transcripts from HS for all transfers. That would cripple you there. BTW, how will you pay for all those private Us? Transfers MAY get financial aid but have few merit aid options.
