<p>Something like,</p>
<p>"Dear Financial Aid Office,
On 06/XX/2013, I received an unrestricted scholarship check from [Donor Name/Organization or Company] in the amount of $500. I have deposited the funds in my checking account.</p>
<p>In addition, in June of 2012 (or whatever date you received it) I received an unrestricted scholarship check in the amount of $200 from [Donor] for the 2012-2013 school year which I deposited in my checking account. At that time I was not aware that I was required to report the scholarship. Please let me know if you need further information.</p>
